If you would like to request a consultation at our clinic, please fill out and submit this form so we can create a file for you and your pet. Once the file has been created, a member of our Reception Team will be in contact with further information and booking details for our initial consultations.

The Dermatology Follow Up Questionnaire is no longer needed, but we encourage you to continue filling out your pet’s Progress Monitoring Sheets and bringing these with you to your next appointment. You can also print off more Monitoring Sheets for treatment & progress tracking from here when needed.
Telemedicine consultations are only available for existing clients and patients. To book your follow up teleconsult, we must have the Consent Form signed and submitted for your file. We also advise having your pet’s Progress Monitoring Sheets with you during your phone or video call. Print out versions are available here.
