Request a Consultation

Would your pet benefit from a veterinary dermatology specialist appointment? If you would like to request a consultation at our clinic, please fill out and submit this form so we can create a file for you and your pet. Once the file has been created, a member of our Reception Team will be in contact with further information and booking details for our initial consultations.

Preparing for Your Pet’s Initial Consultation

First time coming in for an appointment? Please fill out and submit the following forms at least three days before your booked initial consultation.

Returning for Your Pet’s Follow Up Consultation

The Dermatology Follow Up Questionnaire is no longer needed, but we encourage you to continue filling out your pet’s Progress Monitoring Sheets and bringing these with you to your next appointment. You can also print off more Monitoring Sheets for treatment & progress tracking from here when needed.

Booking a Follow Up Telemedicine Consultation

Telemedicine consultations are only available for existing clients and patients. To book your follow up teleconsult, we must have the Consent Form signed and submitted for your file. We also advise having your pet’s Progress Monitoring Sheets with you during your phone or video call. Print out versions are available here.

Booking a Telemedicine Consultation

If you have been in contact with our clinic about booking a Telemedicine Consult with our veterinary dermatologist, please fill out and submit the required forms below. Payment details will follow.

We look forward to assisting you and your family with professional, compassionate, and individualized pet care with regards to dermatologic and allergic concerns.

In consideration of other clients, their pets, and your vet dermatologist, please allow at least 72 hours’ notice to change or cancel appointments.
